Usage is measured by the source words processed for translation, including the prompt text. Adding more target languages increases your total words processed, but the billing metric remains words in, not languages out.

No. Forthwith compares your source strings and only sends new or changed entries for translation. Ongoing monthly usage is typically much lower than an initial full-project run.
